REGISTER?

All newly registered patients are asked to see the doctor or practice nurse for a medical check when joining the practice. You will be given a health questionnaire to complete which helps us to care for you whilst awaiting receipt of your medical records. This is also your chance to get to know your surgery, your doctor and nurse.

It is extremely important to let us know immediately if your personal details change, ie name, address etc, otherwise confusion can arise over medical records and correspondence can go astray.
