Patient Requests
Please allow up to 30 working days excluding weekends for your request to be completed.
We ask that patients complete the online form to ensure all the relevant details are received to avoid any unnecessary delay. We do not accept requests via telephone.
This form can be used for the following requests:
- General letter request
- Fit to Fly Letter
- Copy of medical records
- Holiday Cancellation Letter/Form
- Firearms Report
- Private referral Letter
- Fit to work/School Letter
- Other , please specify
If you require forms to be completed then we would ask that you either bring these to the surgery or email directly to the secretaries (secretary.thechestnutssurgery@nhs.net).
A member of our team will be in touch once this request has been completed. Please be aware that these requests do not take priority over NHS work and there will be a charge of collection, which may vary.
You will be contacted by the surgery if we require any further details either via email or via phone.

All Information submitted through secure forms is secured with a private key and is accessed over a secure
connection by nominated staff. We have a strict confidentiality policy.
